YOUR ROLE
As the Program Manager of Chronic and Complex Patient Management (CCPM), you will oversee a team dedicated to providing longitudinal care management for patients with complex and chronic conditions, including kidney disease, diabetes, heart failure, and other comorbidities. You will serve as a collaborative and supportive leader responsible for the day-to-day operations of your care management team, focusing on proactive, patient-centered care to manage chronic diseases effectively. By leveraging expertise and experience in managing patients with chronic and complex needs, you will ensure care pathways are followed to minimize complications, prevent hospital admissions, and enhance patients' quality of life. You will also manage CCPM program performance and provide continuous training and development to ensure Evergreen’s key performance indicators (KPIs) and performance metrics meet or exceed expectations.
PRIMARY FUNCTIONS
Overseeing the daily operations of the CCPM team to ensure efficient and effective disease management and performance goal achievement.

Continuously evaluating and improving the performance of the CCPM program and team members, consistent with Evergreen’s performance management philosophy.
Developing and implementing collaborative strategies to improve program performance, reduce readmissions, and ensure patient and provider partner satisfaction.
Supporting newly hired CCPM team members through orientation and onboarding, as well as providing ongoing training and development for established team members.
Completing routine audits to drive performance management and improvement.
Preparing performance reports and meeting materials on the CCPM program for both internal and external audiences.
Developing and fostering relationships with key stakeholders, including members of the interdisciplinary teams (IDTs) and other program leaders, across the organization to drive outcome achievement through collaboration and cross-program partnership.
Building and maintaining effective communication strategies between Evergreen and market stakeholders to foster strong relationships that support and ultimately achieve improved patient outcomes, in collaboration with market leaders.

HOW YOU QUALIFY
You have a Bachelor’s degree in healthcare or nursing (BSN required)
Master's Degree in Nursing or Healthcare Administration or related field preferred
You have an active state nursing license in good standing
Minimum of 3 years of longitudinal clinical nursing care management experience required
Minimum of 2 years' experience in a clinical nursing managerial or leadership role
Experience in longitudinal care management is required
Experience with quality improvement using data to track and improve program performance strongly preferred
You have an understanding of and experience in Value Based Care
You understand Business Continuity and how to build resiliency into business processes / systems You develop talent, know when to delegate, innovate and iterate with your team, leveraging their strengths
You have excellent relationship, partnership building, communication, change management and influencing skills, with the ability to build trust and respect quickly, both internally and externally

You have previous experience working within a managed care environment especially with experience in administrative/management capacity
You have experience in leading the clinical function of a Value Based Care organization
Intermediate skills with MS Office Suite of products including Outlook and Teams
You have the tenacity required to drive initiatives forward, overcome obstacles, and achieve goals despite challenges
You have the ability to work effectively in a primarily remote environment:
At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wired to the house internet (Cable, Fiber, or DSL) and hardwired to the internet device is recommended
Evergreen will provide Remote or Hybrid Home/Office employees with telephony applications and equipment to meet the business requirements for their position/job
Team Members must work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
Compensation
The pay range for this role is $113,000 to $120,000 annually. Exact pay is determined based on experience, education, demand for the role, and other role-specific factors.
