The eviCore Hospital Liaison is responsible for coordinating the transition of care from the acute care setting to post-acute for patients referred to post-acute services. Areas of focus for the Hospital Liaison will be enhancing patient and family satisfaction, streamlining communication, and building relationships with hospital-based referral sources representing the company as the leader in post-acute care management to all referral sources. Establishes good working relationships with social workers, nurses, physicians, and case managers in assigned hospitals and other medical facilities and acts as the first point of contact (liaison) for the hospital staff when a referral is made for post-acute services. Educates clients, family members, significant others, referral sources, hospital personnel, and external case managers about eviCore healthcare's programs and providers. Educates discharge planners, social workers, nurses, and physicians regarding eviCore healthcare programs and services provided via in-service training. Keeps leadership abreast of any competitor issues that would affect referrals within the market. Other duties as assigned.
Minimum Experience Required
LPN, MSW, or BSW preferred
Home Health, SNF, and/or rehab experience desired
2 to 3 years of outside sales and/or marketing in healthcare
Experience with understanding Medicare, Medicaid, and Managed care post-acute guidelines
Position is located in onsite facilities in the New York City area.
Daily travel between hospitals in the greater New York City area; occasional travel to New York City post-acute facilities
Position is full-time during core business hours with the ability to work any shift between 7am and 6pm CST (8am-7pm EST), with evening call availability and weekend call rotation
Dependable and reliable work schedule is essential
Requires use of own vehicle or mode of transportation for travel >75% of time to onsite facilities and onsite participation in multi-disciplinary meetings
The ability to maintain normal, reasonable levels of personal hygiene and professional appearance

Communication: Ability to provide clear verbal and written information via in-person, email, and phone to customers, co-workers, and leadership

Proficient in computer literacy skills with ability to use and troubleshoot Microsoft Word, Excel, Outlook, and PowerPoint with ability to type 30 wpm preferred
Experience using videoconferencing systems such as WebEx
Must be able to read, write and speak the English language; 2nd language preferred
Complies with federal and eviCore healthcare requirements regarding protected health information and patient privacy
Must maintain wired internet connection in the home (i.e. cable/DSL) and demonstrate minimum Speeds of: Upload: 2 mbps, and Download: 10 mbps.
